and then cut them up so that each Certificate is pocket size
and will fit easily inside of a small New Testament Bible.
However, you are welcome to make your Certificate any size you wish!

When You Lead Someone To Christ...

Give Them A "New Birth Certificate"
And Write in the Date that they Accepted Christ as Saviour on the Top Line

Then Write their FULL NAME on the Second Line

Go through EACH of the Assurance verses* on the New Birth Certificate with your convert!

(*Depending upon time...You may have to set up a time to come back in order to do this thoroughly!)

Help that "new babe in Christ" to understand that they now belong to Christ FOREVER!

Give them the New Birth Certificate to keep with them
so as to remind them of the date that they placed their faith
in Jesus Christ as their own personal Lord and Saviour!

*NOTE: You should add the Name of your Church, along with the correct Address, Phone Number, and other important information on your ministry's New Birth Certificates!

**Give them YOUR Name and telephone number in case they have any questions!**

2: Get Something from them so that YOU Remember The Date
They Accepted Jesus Christ as their Saviour
so YOU can FOLLOW-UP on them and help them to grow in Christ!

Get as much information as you can so that you may better follow-up on your convert.

This will help determine which Sunday School Teacher should visit them
and whether a male or female follow-up person should come and visit your convert as well!

Keep one copy for your records and give the other copy to the other teachers or follow-up people!

If the person you lead to Christ is a child and a parent or legal guardian gives you permission
to bring them to Church on the Bus, them use this 3-part form to get information on the child
and a parent's or legal guardian's signature giving you their permission for their child
to ride your bus to Church on Sunday.

Please note...this is a 3-part, Double Sided Form.

The BACK SIDE of the form should always be read to the parents or legal guardian.
It explains to them your purpose in bringing their child to Church on your Sunday School/Church Bus.
This form is VERY IMPORTANT in today's wicked society of child molesters.
You want to help that parent or guardian understand "who you are" and "where you are going"
with their child as well as "what their child will be taught" when they get to your Church!

It also opens a door for you to explain God's Plan of Salvation to the parents or guardians!

Once they have read and understand the reason for the form and what your intentions are,
then ask them to sign THE FRONT PAGE OF THE FORM. After they sign the form,
then YOU write YOUR NAME, YOUR BUS NUMBER, AND YOUR HOME OR CELL PHONE
NUMBER at the top of the form and tell them this is how they can reach YOU at the Church
in the event of an emergency!

Also write in the Pick Up Time that you will come by their house and pick up their child for Church!

Then Tear out the CENTER YELLOW COPY and give to them to keep for their informational purposes!

Give the ORIGINAL to your Bus Director or your Sunday School Superintendent or Church Secretary
so that there is a copy of the information on record n the Church Office!

You will need to keep the BOTTOM CARDSTOCK COPY in your Bus Book for your Bus Records!

4: Keep up with who rides your Bus from week to week with this simple form!

This Bus Rider Information Sheet can be easily be copied on 8-1/2" by 11" paper
and provides space for 64 riders!
It also provides an area to keep up with who brought their Bible, and who brought a Visitor
and the age of each Rider.
There is space at the Top of each form for the Captain 's Name, Bus Number and Date.

The form is designed to fit on a standard Clipboard so there is space at the top of the form for that as well!

After you write down each Bus Rider's name on your Bus Roll
MAKE CERTAIN you mark your Bus Number on the BACK of each rider's hand
especially if they are under the age of 18!

Each driver will use this form when he or she does their "pre-trip" check on their bus in the morning. Any problems found should be noted on the inspection form at that time. This form also provides an area to write down any problems found on the afternoon return home trip as well.
